ABSTRACT

ABSTRACT Malnutrition/sarcopenia is frequent in patients with inflammatory bowel diseases (IBD), and results in muscle catabolism, impacting treatment response, postoperative complications, and quality of life. Objective: This study aims to assess whether the phase angle (PhA) is a parameter for predicting reduced muscle mass in patients with IBD. Methods: Adult patients with IBD were included in this cross-sectional study. For the estimation of muscle mass and the calculation of the PhA, we used bioelectrical impedance analysis (BIA). Crohn's disease (CD) and ulcerative colitis (UC) activity scores were defined using the Harvey-Bradshaw index and partial Mayo score, respectively. The area under the ROC curve was calculated to identify the PhA cut-off point for reduced muscle mass. Results: The sample consisted of 145 patients, with 39 (26.9%) with IBD in the active phase. There was a correlation of the PhA with skeletal muscle mass (SMM) (rs 0.35, P<0.001) and with the skeletal muscle mass index (SMI) (rs 0.427, P<0.001), and the associations remained in the most active form (moderate or severe) of IBD. The ROC curve analysis indicated that the cut-offs points of the PhA ≤5.042 for female and PhA ≤6.079 for male can be used to predict muscle mass reduction. Conclusion: The PhA can be considered a predictor of muscle mass reduction in IBD patients, and we can use it for screening and monitoring the evolution of malnutrition.


RESUMO A desnutrição/sarcopenia é frequente em pacientes com doenças inflamatórias intestinais (DII), resultando em catabolismo muscular, com impacto nas respostas aos tratamentos, complicações cirúrgicas e na qualidade de vida. Objetivo: Este estudo tem como objetivo, avaliar se o ângulo de fase (AF) é um parâmetro para a predição de redução de massa muscular em pacientes com DII. Métodos: Pacientes adultos com DII foram incluídos neste estudo transversal. A estimativa da massa muscular e o cálculo do AF foram realizados a partir do exame de bioimpedância elétrica (BIA). As atividades da doença de Crohn e retocolite ulcerativa foram definidas pelo índice Harvey-Bradshaw e escore parcial de Mayo, respectivamente. A área de curva ROC foi calculada para identificar o ponto de corte do AF para a massa muscular reduzida. Resultados: A amostra foi composta por 145 pacientes, sendo 39 (26.9%) com DII em fase ativa. Houve correlação do AF com massa muscular esquelética (MME) (rs 0.35, P<0.001) e com o índice de massa muscular esquelética (IMME) (rs 0.427, P<0.001), mantendo-se as associações na forma mais ativa (moderada ou grave) da DII. A análise da curva ROC indicou que os pontos de corte de AF ≤5.042 para mulheres e ≤6.079 para homens podem ser usados para prever a redução da massa muscular. Conclusão: O AF pode ser considerado um preditor de redução de massa muscular nos pacientes com DII e ser utilizado para triagem e acompanhamento da evolução da desnutrição.

ABSTRACT

O objetivo deste estudo foi analisar a tendência temporal da desnutrição em crianças menores de 5 anos de idade assistidas pelo Programa Bolsa Família entre 2008 e 2019, explorando desigualdades regionais e buscando determinar o impacto das crises econômica e política agravadas em 2014 e da adesão governamental às políticas de austeridade fiscal na tendência. As análises foram realizadas utilizando dados agregados de lactentes (0-23 meses) e pré-escolares (24-59 meses), extraídos do Sistema de Vigilância Alimentar e Nutricional (SISVAN) assistidas pelo Programa Bolsa Família (n = 34.272.024). As tendências foram analisadas por meio de modelos lineares generalizados, com efeitos mistos específicos para as faixas etárias (distribuição binomial negativa e função de ligação log). As desigualdades regionais foram analisadas a partir do agrupamento das Unidades Federativas segundo o Índice de Vulnerabilidade Social (IVS) e a influência das crises e das políticas de austeridade na prevalência de desnutrição por meio da interação entre "ano" e "crise" (2008-2013 vs. 2014-2019). Houve redução na prevalência de desnutrição infantil até meados de 2013, quando as tendências passaram a ser estacionárias para pré-escolares e ascendentes para lactentes. Observou-se, também, maior risco de desnutrição nos estados com média e alta vulnerabilidade social, quando comparadas àqueles com baixa vulnerabilidade social. Os pontos de inflexão nas tendências corroboram a hipótese de que as crises política e econômica, e as respostas governamentais a essas crises, provocaram impacto negativo sobre o estado nutricional de crianças em situação de pobreza e extrema pobreza no Brasil.


This study aimed to analyze the temporal trend of malnutrition in children aged under five years assisted by the Brazilian Income Transfer Program from 2008 to 2019, by exploring regional inequalities and seeking to determine the impact of the economic and political crises aggravated in 2014, and the government's adherence to fiscal austerity policies on the trend. The analyses were performed using aggregated data from infants (0-23 months) and preschoolers (24-59 months), extracted from the Brazilian Food and Nutritional Surveillance System (SISVAN) assisted by the Brazilian Income Transfer Program (n = 34,272,024). Trends were analyzed using generalized linear models with age-specific mixed effects (negative binomial distribution and log linkage function). The regional inequalities were analyzed based on the grouping of Federative Units according to the Social Vulnerability Index (SVI) and the influence of crises and austerity policies on the prevalence of malnutrition by the interaction between "year" and "crisis" (2008-2013 vs. 2014-2019). There was a reduction in the prevalence of child malnutrition until mid-2013, when the trends became stationary for preschoolers and upward for infants. There was also a higher risk of malnutrition in Federative Units with medium- and high-social vulnerability, when compared to those with low-social vulnerability. The inflection points in the trends corroborate the hypothesis that the political and economic crises, and the governmental responses to these crises, negatively impacted the nutritional status of children in poverty and extreme poverty in Brazil.


El objetivo de este estudio fue analizar la tendencia temporal de la desnutrición en niños menores de cinco años atendidos por el Programa Bolsa Familia entre los años 2008 y 2019, explorando desigualdades regionales y buscando determinar el impacto de las crisis económica y política que se intensificaron en 2014, así como la adhesión del gobierno a políticas de austeridad fiscal en esta tendencia. Los análisis se realizaron utilizando datos agregados de lactantes (0-23 meses) y preescolares (24-59 meses), extraídos del Sistema de Vigilancia Alimentaria y Nutricional (SISVAN) atendidos por el Programa Bolsa Familia (n = 34.272.024). Se analizaron las tendencias a través de modelos lineales generalizados con efectos mixtos específicos para los grupos de edad (distribución binomial negativa y función de enlace de logaritmo). Se analizaron las desigualdades regionales a partir del agrupamiento de las unidades federativas conforme el Índice de Vulnerabilidad Social (IVS) y la influencia de las crisis y de las políticas de austeridad en la prevalencia de desnutrición a través de la interacción entre "año" y "crisis" (2008-2013 vs. 2014-2019). Hubo una disminución en la prevalencia de desnutrición infantil hasta mediados de 2013, cuando las tendencias se volvieron estacionarias para preescolares y ascendentes para lactantes. También se observó un riesgo más alto de desnutrición en estados con vulnerabilidad social media y alta, en comparación con aquellos con vulnerabilidad social baja. Los puntos de inflexión en las tendencias corroboran la hipótesis de que las crisis política y económica, y las respuestas del gobierno para estas crisis, tuvieron un impacto negativo en el estado nutricional de niños en situación de pobreza y extrema pobreza en Brasil.

Abstract Background Epilepsies are among the most prevalent chronic neurological diseases, usually beginning in childhood. About 30% of children with epilepsies develop seizures that are difficult to control with medication. Recurrent epileptic seizures hinder diet intake, impairing the nutritional status. Although non-pharmacological interventions (e.g., ketogenic diet therapy) can improve epileptic seizure frequency, few studies analyzed their impact on the nutritional status of children and adolescents with epilepsies. Objective The aim was to evaluate the effects of a ketogenic diet on the nutritional status and clinical course of patients with pharmacoresistant epilepsies. Methods This cross-sectional study included patients under 18 years of age followed up at the Ketogenic Diet Ambulatory Clinic of the Instituto de Medicina Integral Prof. Fernando Figueira between December 2015 and December 2021. Socioeconomic, clinical, nutritional, and laboratory data were collected from medical records at different time points during the ketogenic diet. Results The sample comprised 49 patients aged between 5 months and 17 years (median = 4.4 years), mostly male (62.1%), and from Recife and the metropolitan region (51%). Underweight patients (BMI-for-age) improved their nutritional status in six months. However, patients who were normal weight and overweight maintained their nutritional status. Dyslipidemia was a common and short-term adverse effect. Moreover, the treatment decreased epileptic seizure frequency and antiseizure medication intake. Conclusion The ketogenic diet prevented malnutrition from worsening and reduced epileptic seizures and antiseizure medication intake.


Resumo Antecedentes A epilepsia, uma das doenças neurológicas crônicas mais prevalentes, tem geralmente início na infância. Cerca de 30% das crianças com epilepsia desenvolvem crises de difícil controle medicamentoso. As crises epilépticas recorrentes dificultam a ingestão alimentar, prejudicando o estado nutricional. Intervenções não farmacológicas, como a terapia com dieta cetogênica, podem melhorar a frequência das crises epilépticas, mas existem poucos estudos sobre a repercussão no estado nutricional da criança/adolescente. Objetivo Avaliar o efeito da terapia cetogênica sobre o estado nutricional e a evolução clínica da epilepsia fármaco-resistente. Métodos Estudo tipo corte transversal envolvendo menores de 18 anos acompanhados no Ambulatório de Dieta Cetogênica do Instituto de Medicina Integral Prof. Fernando Figueira entre dezembro de 2015 e dezembro de 2021. Dados socioeconômicos, clínicos, nutricionais e laboratoriais foram coletados nos prontuários dos pacientes em vários momentos da terapia cetogênica. Resultados A amostra foi composta por 49 pacientes com idades entre cinco meses e 17 anos (mediana = 4,4 anos), a maioria do sexo masculino (62,1%) e procedentes de Recife e região metropolitana (51%). Pacientes com baixo peso (de acordo com o IMC para idade) melhoraram seu estado nutricional em seis meses. No entanto, os pacientes com peso adequado e com sobrepeso mantiveram seu estado nutricional. A dislipidemia foi um efeito adverso frequente e de curta duração. Além disso, o tratamento reduziu a frequência de crises epilépticas e a dose de fármacos anticrises. Conclusão A dieta cetogênica preveniu o agravamento da desnutrição e reduziu as crises epilépticas e a dosagem de fármacos anticrises.

ABSTRACT

Background:Moderate acute malnutrition (MAM) is a leading cause of childhood morbidity and mortality globally. The morbidity pattern of underfives with this condition is yet to be described. Objective: To describe the morbidity pattern of underfives with MAM.Method: Across sectional study was conducted in two Primary Health Centres in Uruan Local Government Area of Akwa Ibom State. Caregivers'brought children aged 6- 59 months to the health facilities following community mobilization. Eligible children were recruited into the study after obtaining parental consent. Avalidated proforma was used to obtain the biodata and symptoms of common illnesses in the children. Ageneral physical examination, anthropometric measurements and systemic examination were performed. Results: Atotal of 162 children were recruited into the study. Their mean (±SD) age was 20.4 ± 13.0 months. Over 70% of them were 6 - 23 months of age. Their mean (±SD) length/height was 77.3 ± 29.6 cm, mean (±SD) weight was 8.3 ± 3.4 kg and mean (±SD) mid upper arm circumference was 12.4 ± 4.5 cm. The main symptoms noted in the children were; fever 99 (61.1%), cough 84 (51.9%), weight loss 81 (50.0%), diarrhoea 40 (24.7%) and vomiting 40 (24.7%) while pallor 77 (47.5%), lymphadenopathy 56 (34.6%), hair changes 49 (30.2%), skin changes 27 (16.6%) were the main signs in them.Conclusion: The main symptomatology of underfives with MAM were fever, cough and weight loss

ABSTRACT

ObjectiveTo investigate the characteristics of the composition of intestinal flora and the serum level of endotoxin in cirrhotic patients with malnutrition, and to provide new diagnosis and treatment ideas for improving the nutritional status of patients with liver cirrhosis. MethodsA total of 58 patients with liver cirrhosis who were hospitalized in Department of Gastroenterology, People’s Hospital of Zhengzhou, from March 2021 to November 2022 were enrolled as experimental group (LC group), and according to the Royal Free Hospital-Nutritional Prioritizing Tool, they were divided into low malnutrition risk group (LC-A group with 28 patients) and moderate/high malnutrition risk group (LC-B group with 30 patients); 25 individuals who underwent physical examination during the same period of time were enrolled as control group (HC group). Peripheral blood and feces samples were collected from all subjects. The limulus amebocyte lysate gel method was used to measure the concentration of endotoxin in peripheral blood, and high-throughput sequencing and bioinformatics analysis were used to investigate the characteristics of intestinal flora. The independent-samples t test was used for comparison of normally distributed continuous data between two groups; a one-way analysis of variance was used for comparison between multiple groups, and the least significant difference t-test were used for further comparison between two groups. The Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups, and the Kruskal-Wallis H test was used for comparison between multiple groups. The chi-square test was used for comparison of categorical data between groups. The Spearman test was used for correlation analysis. ResultsThere were significant differences between the three groups in the levels of alanine aminotransferase (H=7.054, P<0.05), gamma-glutamyl transpeptidase (H=9.644, P<0.05), albumin (Alb) (F=32.768, P<0.05), total bilirubin (TBil) (H=20.980, P<0.05), and serum endotoxin (F=108.672, P<0.05). There was a significant difference in Chao1 index between the three groups (F=5.110, P=0.008) and between the HC group and the LC-B group (P<0.05). Compared with the HC group, the LC-A group and the LC-B group had significant reductions in Chao1 index and Shannon index, and there was a significant difference in Chao1 index between the HC group and the LC-B group (P<0.05). At the phylum level, the intestinal flora in each group was mainly composed of Bacteroidota, Firmicutes, Proteobacteria, and Actinobacteriota, accounting for more than 95% of all phyla, and there was a significant difference in the relative abundance of Firmicutes between the HC group and the LC-B group (P<0.05). Serum endotoxin was significantly negatively correlated with Ruminococcaceae (r=-0.420, P=0.007), and spirochete was significantly positively correlated with TBil (r=0.419, P=0.007) and was significantly negatively correlated with Alb (r=-0.492, P=0.001). ConclusionThere are unique changes in intestinal flora in cirrhotic patients with malnutrition, and differentially expressed flora are associated with endotoxemia. Improving intestinal microecology in liver cirrhosis may help to improve nutritional status.

ABSTRACT Background: The prevalence of malnourished patients before transplantation and the influence of malnutrition on graft and patient outcomes remain underestimated, despite being associated with higher postoperative morbidity and mortality. This study aimed to develop an easy nutritional screening tool and evaluate the impact of nutritional status on clinical outcome, graft survival (GS) and mortality risk in kidney transplant patients (KTP). Methods: In this retrospective cohort study including 451 KTP, we developed a score by using anthropometric, clinical, and laboratory measures performed in the pretransplant evaluation. The patients were stratified into 3 groups according to the final score: G1 (0 or 1 point)=low risk, G2 (2 to 4 points)=moderate risk, and G3 (>5 points)=high risk of malnutrition. The patients were monitored after transplantation at least 1 to 10 years. Results: Stratifying the 451 patients based on the pretransplant risk score, G1, G2, and G3 were composed of 90, 292, and 69 patients, respectively. Patients from G1 maintained the lowest serum creatinine levels at hospital discharge when compared with others (p = 0.012). The incidence of infection in the patients from G3 was higher than patients from G1 and G2 (p = 0.030). G3 recipients showed worse GS than G1 patients (p = 0.044). G3 patients showed almost threefold higher risk for graft loss (HR 2.94, 95% CI 1.084-7.996). Conclusions: KTP with higher malnutrition risk score were associated with worse outcomes and GS. The nutritional screening tool is easy to be used in clinical practice to evaluate the patient in preparation for kidney transplant.


RESUMO Antecedentes: A prevalência de pacientes desnutridos antes do transplante e a influência da desnutrição nos desfechos do enxerto e do paciente permanecem subestimadas, embora estejam associadas a maior morbimortalidade pós-operatória. Este estudo buscou desenvolver uma ferramenta simples de triagem nutricional e avaliar o impacto do estado nutricional no desfecho clínico, sobrevida do enxerto (SE) e risco de mortalidade em pacientes transplantados renais (PTR). Métodos: Neste estudo de coorte retrospectivo incluindo 451 PTR, desenvolvemos um escore usando medidas antropométricas, clínicas e laboratoriais tomadas na avaliação pré-transplante. Os pacientes foram estratificados em 3 grupos segundo a pontuação final: G1 (0-1 ponto) = baixo risco, G2 (2-4 pontos) = risco moderado e G3 (>5 pontos) = alto risco de desnutrição. Eles foram monitorados por pelo menos 1 a 10 anos após o transplante. Resultados: Os 451 pacientes foram estratificados em G1, G2 e G3, que consistiram em 90, 292 e 69 pacientes, respectivamente. Os pacientes do G1 mantiveram os menores níveis de creatinina sérica na alta hospitalar em relação aos demais (p = 0,012). A incidência de infecção nos pacientes do G3 foi maior que nos pacientes do G1 e G2 (p = 0,030). Os pacientes do G3 apresentaram SE pior do que os pacientes do G1 (p = 0,044) e um risco quase três vezes maior de perda do enxerto (HR 2,94; IC 95% 1,084-7,996). Conclusões: PTR com maior escore de risco de desnutrição foram associados a piores desfechos e menor SE. A ferramenta de triagem nutricional é fácil de usar na prática clínica para avaliar pacientes em preparação para transplante renal.

La desnutrición crónica se constituye en un problema de salud pública que afecta el bienestar de la población infantil. Objetivo. Analizar la situación de la desnutrición crónica y anemia infantil en Perú y las intervenciones de los programas sociales. Materiales y Métodos. Fue un estudio descriptivo cualitativo, de diseño de análisis de contenido. En la recolección de datos la técnica utilizada fue el análisis de documentos, el instrumento fue la matriz de análisis de documento y datos (MDD). Resultados. La desnutrición crónica desde el año 2007 hasta el 2022 ha mostrado un comportamiento de reducción favorable de 28.5% a 11.7%. Y la anemia desde el año 2007 hasta el año 2021 hubo una reducción de 56.8 % a 38.8%, y en el año 2022 fue de 42.4%. En la zona rural se encontró en 51.5%. Conclusiones. La situación de la desnutrición crónica y anemia infantil en Perú disminuyó en 11.7% al año 2022. Siendo la zona rural la más afectada. Los departamentos de Huancavelica, Loreto y Cajamarca ocupan los más altos porcentajes. El porcentaje de la anemia fue 42.4%, y la zona rural supera el 50%; encontrándose con mayor porcentaje los departamentos de Puno, Ucayali, Huancavelica y Loreto. Ello se afirma como problemas latentes y forma parte de los indicadores de la ODS. La intervención de los programas sociales a través de programas nutricionales ha tenido un financiamiento sostenido. Sin embargo, expertos evaluaron el programa chispitas, suplementación con hierro mostrando que no hay un efecto en la disminución de la desnutrición y anemia.


Chronic malnutrition is a public health problem that affects the well-being of the child population. Objective. To analyze the situation of chronic malnutrition and childhood anemia in Peru and the interventions of social programs. Materials and Methods. This was a qualitative descriptive study, with a content analysis design. The data collection technique used was document analysis, the instrument was the document and data analysis matrix (MDD). Results. Chronic malnutrition from 2007 to 2022 has shown a favorable reduction behavior from 28.5% to 11.7%. And anemia from 2007 to 2021 there was a reduction from 56.8% to 38.8%, and in 2022 it was 42.4%. In the rural area it was 51.5%. Conclusions. The situation of chronic malnutrition and child anemia in Peru decreased by 11.7% by the year 2022. The rural area is the most affected. The departments of Huancavelica, Loreto and Cajamarca have the highest percentages. The percentage of anemia was 42.4%, and the rural area exceeds 50%, with the highest percentages in the departments of Puno, Ucayali, Huancavelica and Loreto. This is affirmed as latent problems and is part of the SDG indicators. The intervention of social programs through nutritional programs has had sustained funding. However, experts evaluated the program "chispitas", iron supplementation, showing that there is no effect in reducing malnutrition and anemia.

Os departamentos de Huancavelica, Loreto e Cajamarca têm as porcentagens mais altas. A porcentagem de anemia foi de 42,4%, e a área rural ultrapassa 50%, com as porcentagens mais altas nos departamentos de Puno, Ucayali, Huancavelica e Loreto. Isso é afirmado como um problema latente e faz parte dos indicadores dos ODS. A intervenção de programas sociais por meio de programas nutricionais teve um financiamento sustentado. No entanto, especialistas avaliaram o programa "chispitas", de suplementação de ferro, mostrando que não há efeito na redução da desnutrição e da anemia.

En la actualidad, aunque las tasas de malnutrición que afectan a los pueblos indígenas y a las zonas de bajos ingresos han disminuido en un 33,8%, América Latina sigue registrando niveles alarmantes de malnutrición, especialmente a nivel infantil. En el caso de Perú en particular, urge estudiar el impacto del presupuesto sanitario en la dinámica de la desnutrición infantil. Objetivo. Analizar la incidencia del presupuesto de salud pública en la reducción de la desnutrición infantil en Perú durante las dos últimas décadas. Metodología. Revisión narrativa acerca del impacto de los presupuestos de salud pública en la desnutrición. Dentro de busqueda se consideraron bases de datos como Scopus, Web of Science, Scielo, Redalyc, Latindex y Google Scholar desde el 2019 a 2023. Considerando con ecuación health budget" OR "chronic child malnutrition", "government spending" con la función booleana AND como parámetros de búsqueda para el campo "topic" para encontrar documentos que también trataran sobre "malnutrition" OR "chronic child malnutrition" OR "child malnutrition". Resultados. Se identificaron 57 fuente, se excluyeron 4 artículos por duplicidad y 37 por falta de información, dando como resultando un total de 16 artículos a considerar en este análisis. Conclusión. Se puede concluir que existe una relación significativa entre el aumento del presupuesto público en salud y la disminución de la desnutrición infantil a nivel nacional. La literatura existente destaca que a medida que se incrementa la asignación presupuestaria en el ámbito de la salud, se observa una reducción correspondiente en las tasas de desnutrición.


Currently, although malnutrition rates affecting indigenous peoples and low-income areas have decreased by 33.8%, Latin America continues to register alarming levels of malnutrition, especially at the child level. In the case of Peru in particular, there is an urgent need to study the impact of the health budget on the dynamics of child malnutrition. Objective. To analyze the impact of the public health budget on the reduction of child malnutrition in Peru during the last two decades. Methodology. Narrative review of the impact of public health budgets on malnutrition. Within the search, databases such as Scopus, Web of Science, Scielo, Redalyc, Latindex and Google Scholar from 2019 to 2023 were considered. Considering with equation health budget" OR "chronic child malnutrition", "government spending" with the Boolean function AND as search parameters for the field "topic" to find documents that also dealt with "malnutrition" OR "chronic child malnutrition" OR "child malnutrition". Results. Fifty-seven sources were identified, 4 articles were excluded due to duplicity and 37 due to lack of information, resulting in a total of 16 articles to be considered in this analysis. Conclusion. It can be concluded that there is a significant relationship between the increase in the public health budget and the decrease in child malnutrition at the national level. The existing literature highlights that as the health budget allocation increases, a corresponding reduction in malnutrition rates is observed.


Atualmente, embora as taxas de desnutrição que afetam os povos indígenas e as áreas de baixa renda tenham diminuído em 33,8%, a América Latina continua a registrar níveis alarmantes de desnutrição, especialmente no nível infantil. No caso do Peru, em particular, há uma necessidade urgente de estudar o impacto do orçamento da saúde na dinâmica da desnutrição infantil. Objetivo. Analisar o impacto do orçamento da saúde pública na redução da desnutrição infantil no Peru nas últimas duas décadas. Metodologia. Revisão narrativa do impacto dos orçamentos públicos de saúde sobre a desnutrição. Bases de dados como Scopus, Web of Science, Scielo, Redalyc, Latindex e Google Scholar de 2019 a 2023 foram consideradas na pesquisa. Considerando com equação "health budget" OR "chronic child malnutrition", "government spending" com a função booleana AND como parâmetros de pesquisa para o campo "topic" para encontrar documentos que também tratassem de "malnutrition" OR "chronic child malnutrition" OR "child malnutrition". Resultados. Foram identificadas 57 fontes, 4 artigos foram excluídos devido à duplicação e 37 devido à falta de informações, resultando em um total de 16 artigos a serem considerados nesta análise. Conclusão. Pode-se concluir que existe uma relação significativa entre o aumento do orçamento da saúde pública e a redução da desnutrição infantil em nível nacional. A literatura existente destaca que, à medida que a alocação do orçamento de saúde aumenta, observa-se uma redução correspondente nas taxas de desnutrição.

INTRODUCCIÓN: el síndrome metabólico es una entidad compleja, compuesta por factores de riesgo cardiaco como: Obesidad, Diabetes. Hipertensión, dislipemia etc. OBJETIVO: determinar la frecuencia de síndrome metabólico en adultos mayores de la ciudad El Alto, durante la gestión 2019. METODOLOGÍA: investigación transversal cuantitativa, se evaluaron a 124 individuos, reclutados en Auki Utas por el gobierno autónomo municipal de El Alto, mediante coordinación con la Unidad de Protección al Adulto Mayor, durante la gestión 2019. Previo consentimiento informado, se determinó antropometría, tensión arterial, toma de muestra de sangre venosa, para procesamiento de pruebas bioquímicas, (glucemia, triglicéridos, colesterol total, HDL colesterol y LDL colesterol). RESULTADOS: el 62% (77/124) de la población estudiada se encuadra dentro del diagnóstico de síndrome metabólico, presentando datos alterados en por lo menos tres de los criterios evaluados. CONCLUSIONES: la población estudiada presenta altos porcentajes de síndrome metabólico, con mayor tendencia a la obesidad e hipertensión, por lo que es importante la promoción y prevención, ya que el sobrepeso y obesidad son factores de riesgo para la resistencia a la insulina, determinante de los demás criterios (marcadores), actividades de promoción y prevención son necesarias en la ciudad de El Alto, para evitar morbimortalidad por síndrome metabólico


INTRODUCTION: metabolic syndrome is a complex entity, composed of cardiac risk factors such as: obesity, diabetes. hypertension, dyslipidemia, etc. OBJECTIVE: determine a baseline regarding metabolic syndrome in older adults in the El Alto city, which can be used in the future to carry out potential preventive interventions. METHODOLOGY: quali-quantitative research, studied 124 individuals, recruited in Auki Utas by the Autonomous Municipal Government of El Alto city, through coordination with the Protection Unit for the Elderly, during the 2019 administration. Prior informed consent, anthropometry was determined blood pressure, venous blood sampling, for processing biochemical tests (blood glucose, triglycerides, cholesterol, HDL and LDL). RESULTS: 62% (77/124) of the studied population falls within the diagnosis of metabolic syndrome, presenting altered data in at least three of the evaluated entities. CONCLUSIONS: the population studied presents high percentages of metabolic syndrome, with a greater tendency to obesity and hypertension, so promotion and prevention is important, since overweight and obesity are risk factors for insulin resistance, a determinant of the other criteria (markers), promotion and prevention activities are necessary in the El Alto city, to avoid morbidity and mortality due to metabolic syndrome

ABSTRACT

La esclerosis sistémica sin escleroderma es una forma no común de compromiso sistémico de esta enfermedad, con ausencia de manifestaciones cutáneas. Presentamos el caso de una mujer de 71 años con índice de masa corporal (IMC) en 16,9 kg/m2, con historia de hipotiroidismo y estreñimiento crónico, quien acude a emergencia por dolor y distensión abdominal, asociado a náuseas. Se realizó una tomografía abdomino-pélvica encontrándose cámara gástrica colapsada, asas intestinales con algunos segmentos distendidos, con signos de fecalización de asa delgada. La colonoscopía describió dolicocolon severo. Se observaron placas induradas en codo derecho y en ambos glúteos. Los exámenes auxiliares: ANA 1/640 patrón centromérico, anticuerpos anticentrómero = 227 U/mL (valor normal - VN: 0-25), Anti SCL-70 = 1,9 U/mL (VN 0-25) y anti RNP = 0,1 U/mL (VN 0-25). La capilaroscopía de borde ungueal a 200x mostró megacapilares. El diagnóstico clínico fue esclerosis sistémica sin esclerodermia. Se inició tratamiento con dieta rica en fibra y nutrición parenteral por 1 semana. La evolución de la paciente fue favorable.


Systemic sclerosis sine Scleroderma is an uncommon form of systemic compromise of this disease, in the abscense of cutaneous manifestations. We present the case of a 71 years-old woman, body mass index (BMI) in 16.9 kg/m2, with history of hypothyroidism and chronic constipation, who goes to the emergency with pain, abdominal bloating, and nausea. An abdominopelvic tomography was made, reporting a collapse of the gastric chamber, some segments of intestinal loops distended with signs of fecalization in the small intestine. A colonoscopy described severe dolicocolon. We observed indurated plaques in left elbow and in both buttocks. Lab tests: ANA 1/640 with anticentromeric pattern, anticentromere antibodies = 227 U/mL (0-25), Anti-SCL-70 = 1.9 U/mL (0-25), anti-RNP = 0.1 U/mL (0-25). Peri-ungueal cappilaroscopy 200x showed mega capillaries. Clinical diagnosis was systemic sclerosis sine scleroderma. We initiated treatment with a high-fiber diet and parenteral nutrition for a week. The patient evolution was favorable.

Introducción. En los refrigerios escolares se ha incrementado el consumo de los productos industrializados y se ha observado una disminución en la ingesta de alimentos frescos. Objetivo. Determinar la prevalencia y tendencia de sobrepeso, obesidad y consumo de alimentos no recomendables en escolares del norte de México. Materiales y métodos. Estudio descriptivo y transversal, realizado del año 2015 a 2022 en una muestra de 468 niños de ambos sexos de 9 a 12 años de escuelas públicas, se les realizó mediciones antropométricas de peso, estatura y circunferencia de cintura. Para diagnosticar el estado nutricional se utilizó el puntaje Z del índice de masa corporal (IMC) por medio del software Anthro Plus. El consumo alimentario de los escolares se evaluó por medio de una lista de cotejo durante cinco días consecutivos por observación directa los alimentos y bebidas que los escolares llevaron de casa para consumir durante el horario escolar. Resultados. Se encontró que el sobrepeso (SBP) y la obesidad (OB) aumentó durante la pandemia, siendo mayor el SBP en las niñas (17,0%) y mayor la OB en los niños (46,8%). En cuanto a la ingesta alimentaria se observó que durante la pandemia aumentó el consumo de alimentos no recomendables asociándose con el IMC. Conclusiones. El sobrepeso y la obesidad aumentó durante la pandemia, siendo mayor el sobrepeso en las niñas y mayor la obesidad en los niños. En cuanto a la ingesta alimentaria se observó que durante la pandemia el consumo de alimentos no recomendables específicamente en las bebidas azucaradas aumentó y se asoció con el IMC(AU)


Introduction. School snacks have increased consumption of industrialized products and a decrease in the intake of fresh food has been observed. Objective. Determine the prevalence and trend of overweight, obesity and consumption of undesirable foods in schoolchildren in northern Mexico. Materials and methods. A descriptive and cross-sectional study was conducted in a sample of 468 children of both sexes aged 9 to 12 in public schools and anthropometric measurements of weight, height and waist circumference were made. The body mass index (BMI) Z score was used to diagnose nutritional status using the Anthro Plus software. Food consumption of schoolchildren was evaluated by means of a checklist for five consecutive days by direct observation of the food and beverages that schoolchildren took from home to consume during school hours. Results. Overweight (PBS) and obesity (OB) were found to increase during the pandemic, with higher PBS in girls (17.0%) and higher OB in boys (46.8%). About food intake, it was observed that during the pandemic consumption of undesirable foods increased in association with the BMI. Conclusions. Overweight and obesity increased during the pandemic, being more overweight in girls and obesity in boys. As for food intake it was noted that during the pandemic consumption of foods not specifically recommended in sugary drinks increased and was associated with BMI(AU)

ABSTRACT

Objetivos: Describir la prevalencia del riesgo de deficiencia o exceso en la ingesta de nutrientes y el patrón de consumo de alimentos de acuerdo con las recomendaciones establecidas por el Ministerio de Salud y Protección Social y el Instituto Colombiano de Bienestar Familiar. Materiales y métodos: Se realizó un estudio transversal descriptivo en mujeres sanas que fueron atendidas en dos hospitales del oriente antioqueño, con seguridad alimentaria y nutricional (SAN) según la Escala Latinoamericana y Caribeña de Seguridad Alimentaria (ELCSA), en el primer trimestre de lactancia, evaluadas entre los años 2021 y 2022. La fuente de información fue la historia clínica. Se midieron las características sociodemográficas, clínicas, antropométricas y la ingesta dietética y de micronutrientes. Se empleó estadística descriptiva. Se presenta la prevalencia de déficit o exceso de ingesta calórica y el índice de relación de consumo (IRC). Se utilizaron los software EVINDI v5, PC-SIDE v1.0, Stata 16.1 y Jasp 0.16.4. Resultados: Se incluyeron 30 mujeres lactantes. La prevalencia de riesgo de deficiencia en la ingesta usual de energía fue 43 % y exceso 16 %; el riesgo de deficiencia proteica fue 98 %. El consumo superior al valor de referencia para grasa saturada fue 86 % y carbohidratos simples 72 %. El patrón de consumo se caracterizó por superar las recomendaciones para azúcares (IRC = 1,29), leche y derivados (IRC = 1,09), cereales, raíces, plátanos y tubérculos (IRC = 1,04). No cumplieron las recomendaciones en la ingesta de grasas (IRC = 0,70), carnes, huevos, leguminosas, frutos secos y semillas (IRC = 0,49), frutas y verduras (IRC = 0,41). Conclusiones: El patrón alimentario identificado dista de las guías nacionales, lo que limita el consumo de macro y micronutrientes, y contribuye al círculo intergeneracional de la malnutrición. Es fundamental realizar nuevas investigaciones en el país para identificar otros patrones de consumo e impulsar acciones de política al respecto.


Objectives: To describe the prevalence of the risk of under or overnutrition and the food intake pattern vis á vis the recommendations of the Ministry of Health and Social Protection and the Colombian Family Welfare Institute. Material and methods: Descriptive cross-sectional study carried out in healthy women receiving care in two hospitals of eastern Antioquia, with food and nutrition security in accordance with the Latin American and Caribbean Food Security Scale (ELCSA), in the first trimester of breastfeeding, assessed between 2021 and 2022. The clinical record was used as the source of information. The sociodemographic, clinical, anthropometric characteristics, as well as dietary and micronutrient intake, were measured. Descriptive statistics were used. The prevalence of deficient or excessive energy intake and the food consumption score (FCS) are presented. The EVINDI v5, PC-SIDE v1.0, Stata 16.1 and Jasp 0.16.4 software packages were used. Results: Overall, 30 breastfeeding women were included. The prevalence of the risk of deficient energy intake was 43 %, while the risk of excessive intake was 16 % and the risk of protein deficiency was 98 %. Intake exceeding the reference value for saturated fats was 86 %, and 72 % for simple carbohydrates. The consumption pattern was characterized by exceeding the recommendations for sugars (FCS = 1.29), milk and dairy products (FCS = 1.09), grains, roots, plantains and tubers (FCS = 1.04). Recommendations for the intake of fats (FCS = 0.70), meats, eggs, legumes, nuts, seeds (FCS = 0.49), fruits and vegetables (FCS = 0.41) were not met. Conclusions: The food intake pattern identified is far from meeting the national guidelines, limiting macro and micronutrient intake and contributing to the intergenerational malnutrition cycle. Additional research in the country is essential in order to identify other intake patterns and drive political action.

Resumen El síndrome de kwashiorkor se caracteriza por malnutrición proteica y edema generalizado; algunos de los factores de riesgo que se asocian a su aparición son: vivir en pobreza, el destete reciente, las infecciones y las dietas basadas en maíz y arroz. Este síndrome puede generar manifestaciones cutáneas como piel delgada y seca, hiperpigmentación, áreas confluentes de descamación, cabello seco, hipopigmentado y desprendible, así como una dermatitis erosiva con predominio en pliegues cutáneos. El diagnóstico se basa principalmente en una evaluación nutricional integral, exploración física y estudios de laboratorio, y el éxito del tratamiento se basa en la rehabilitación nutricional temprana. Caso clínico: lactante del sexo femenino de 8 meses de edad, que acudió al Instituto Nacional de Pediatría (INP), por presentar una dermatosis generalizada de tipo descamativa de 1 mes de evolución, que fue tratada con ketoconazol tópico. Al interrogatorio la madre refiere alimentación exclusiva con atole de maíz por un diagnóstico de "alergia a la leche" y falta de recursos económicos para comprar la fórmula hidrolizada. La paciente presentaba una dermatosis diseminada que afectaba todos los segmentos corporales, caracterizada por placas hiperpigmentadas, bien definidas, de forma irregular, con descamación en láminas gruesas en región perioral y extremidades, así como áreas erosionadas, pálidas y edema generalizado en extremidades. Se realizaron exámenes de laboratorio que mostraron que la paciente tenía anemia (Hb 11.2 g/dL) e hipoalbuminemia (3.3 g/dL) que, en conjunto con las manifestaciones clínicas, integraron el diagnóstico de síndrome de Kwashiorkor. Se informaron los hallazgos clínicos y de laboratorio al servicio de Gastroenterología y Nutrición, quienes realizaron una valoración nutricional integral y decidieron iniciar tratamiento nutricional; por parte del servicio de Dermatología, se indicó el uso de emolientes y cuidados generales de la piel. Veinte días después la dermatosis y el edema habían remitido.


Abstract Kwashiorkor syndrome is characterized by protein malnutrition and edema, risk factors are recent weaning, infections, and diets based on corn and rice. This malnutrition can lead to skin manifestations such as thin, dry skin, hyperpigmentation, confluent areas of scaling, dry, hypopigmented, and detachable hair, as well as erosive dermatitis, predominantly in skin folds. The diagnosis is based on a nutritional evaluation exam, physical examination and laboratory finding, the treatment is based on early nutritional rehabilitation. Clinical case: 8-month-old female infant who attended the Instituto Nacional de Pediatría, presenting a scaling dermatosis of 1 month's evolution that was treated with topical ketoconazole. The mother reported exclusive feeding with corn gruel due to the diagnosis of "lactose allergy" and commented not enough resources to buy hydrolyzed formula. The patient presented a disseminated dermatosis to all body segments, characterized by well-defined, irregularly shaped, hyperpigmented plaques with scaling in thick sheets in the perioral region and extremities, as well as areas of eroded skin and paleness and edema of extremities. Laboratory tests were taken, where anemia (Hb 11.2 g/dl) and hipoalbuminemia (3.3 g/dl) were documented, the diagnosis of kwashiorkor syndrome was integrated. The clinical and laboratory findings were reported to the Gastroenterology and Nutrition service, who performed a nutritional assessment, and began nutritional treatment, emollients and general skin care were documented; twenty days later, the dermatosis and edema had subsided.

ABSTRACT

La desnutrición como enfermedad de origen social es la expresión última de la situación de inseguridad alimentaria y nutricional de una población, al afectar principalmente a los niños. El objetivo fue analizar la prevalencia y factores relacionados con la desnutrición en la primera infancia en Colombia durante los años 2018 a 2020, mediante un estudio cuantitativo descriptivo de tipo ecológico - exploratorio, con recolección de datos retrospectivos a partir de reportes obtenidos del Sistema Integrado de Información de la Protección Social. El total de casos corresponde a 43.823 reportes, la prevalencia para los tres años fue de 1,13% principalmente en los departamentos de Guajira (n: 3.488; 3,17%) y Boyacá (n: 1.277; 1,39%), mayor número de casos presentados en el sexo masculino (n: 23.804; 54,3%), en edad entre 0 y 1 año (n: 17.099; 39,0%), pertenecientes al régimen subsidiado (n: 28.814; 65,75%) y ubicados en la cabecera municipal (n: 28.114; 64,15%). Con relación a la pertenencia étnica la mayor frecuencia se evidencia en "otras etnias" (n: 33.050; 75,42%), seguido de la etnia indígena (n: 8.348; 19,05%) y el estrato socioeconómico más representativo es el "bajo-bajo" (n: 17.620; 40,21%). Además, existe relación entre el sexo masculino y la desnutrición, comportándose como un factor de riesgo, y el vivir en centro poblado disminuye la probabilidad de presentar desnutrición. Se evidenció una frecuencia significativa de características asociadas a los determinantes sociales en salud y variables específicas relacionadas con la desnutrición.


Malnutrition as a disease of social origin is the ultimate expression of the situation of food and nutritional insecurity of a population, mainly affecting children. The objective was to analyze the prevalence and factors related to malnutrition in early childhood in Colombia during the years 2018 to 2020, through a descriptive quantitative study of an ecological-exploratory type, with retrospective data collection from reports obtained from the Integrated System of Social Protection Information. The total number of cases corresponds to 43,823 reports, the prevalence for the three years was 1.13%, mainly in the departments of Guajira (n: 3,488; 3.17%) and Boyacá (n: 1,277; 1.39%). greater number of cases presented in males (n: 23,804; 54.3%), aged between 0 and 1 year (n: 17,099; 39.0%), belonging to the subsidized regime (n: 28,814; 65.75%) and located in the municipal seat (n: 28,114; 64.15%). ABSTRACT

En el embarazo el requerimiento energético y de nutrientes aumentan, el déficit o el exceso puede afectar el estado nutricional de la madre y del neonato. Con el objetivo de determinar la frecuencia de malnutrición y su relación con complicaciones en mujeres embarazadas y sus recién nacidos que acuden al Hospital Distrital de Capiatá del Ministerio de Salud Pública y Bienestar Social, en el mes de febrero a agosto de 2022, se realizó un estudio observacional descriptivo de corte transverso retrospectivo que incluyó a 140 gestantes, de las cuales 64 presentaron complicaciones por malnutrición y 76 no presentaron ninguna complicación. La edad media fue de 26,9±6,6 años con un rango de edad de 15 a 40 años. Los resultados muestran que el 45.7% (n=64) de las gestantes presentaron complicaciones por malnutrición, el 51,6% presentó diabetes gestacional, anemia en el 29,7%, el 7,8% presentó anemia y diabetes gestacional, el otro 7,8% hipertensión arterial, el 1,6% manifestó hipertensión arterial y diabetes gestacional y el otro 1,6% presentó anemia, hipertensión arterial y diabetes gestacional. En relación a la evaluación nutricional al inicio y al final de la gestación en las gestantes con complicaciones hubo una leve disminución en los porcentajes de obesidad de 43,7% a 37,5% y un aumento del bajo peso de 11% a 20,3%. En cuanto al tipo de parto en las gestantes con complicaciones el 51,6% fue por cesárea (n=33), las cuales presentaron diabetes gestacional en el 54,5%, seguido de anemia en el 21,2%. Se comprobó un mayor porcentaje de sobrepeso y obesidad (29,7%) en los recién nacidos de madres con complicaciones. Se pudo evidenciar la frecuencia de malnutrición y complicaciones en casi la mitad de las gestantes, es preciso generar mayores estrategias en salud pública para prevenir la malnutrición y complicaciones en gestantes.


In pregnancy the energy and nutrient requirement increases, the deficit or excess can affect the nutritional status of the mother and the neonate. In order to determine the frequency of malnutrition and their relationship with complications in pregnant women and their newborns who go to the Capiata District Hospital of the Ministry of Public Health and Social Welfare, in the month of February to August 2022, a Descriptive observational study of retrospective cross -section that included 140 pregnant women, of which 64 presented complications for malnutrition and 76 did not present any complication. The Middle Ages was 26.9 ± 6.6 years with an age range of 15 to 40 years. The results show that 45.7% (n = 64) of pregnant women presented complications for malnutrition, 51.6% presented gestational diabetes, anemia in 29.7%, 7.8% presented anemia and gestational diabetes, the other 7.8% arterial hypertension, 1.6% expressed arterial hypertension and gestational diabetes and the other 1.6% presented anemia, arterial hypertension and gestational diabetes. In relation to nutritional evaluation at the beginning and at the end of pregnancy in pregnant women with complications there was a slight decrease in obesity percentages of 43.7% to 37.5% and an increase in low weight of 11% to 20, 3%. As for the type of childbirth in pregnant women with complications, 51.6%was due to caesarean section (n = 33), which presented gestational diabetes in 54.5%, followed by anemia in 21.2%. A higher percentage of overweight and obesity (29.7%) was proven in mother's newborns with complications. The frequency of malnutrition and complications could be evidenced in almost half of the pregnant women, it is necessary to generate greater public health strategies to prevent malnutrition and complications in pregnant women.

Current Indian food system is not sustainable as it fails to fulfil its primary function of delivering adequate nutrition to its population while causing high environmental impacts along with widespread poverty among farmers. Here, we discuss how recent research has enabled quantification of a country’s current food system sustainability through multiple indicators across nutrition, environmental, and economic dimensions. This data can be used by policy makers, farmers, businesses, consumers, and other stakeholders to make scientific evidence-based informed decisions regarding which diets and food items to promote or discourage in near future to make progress towards sustainability. While several government initiatives are underway to transform Indian agri-food sector, the need of the hour is multi-sectoral collaboration across ministries along with dietary behavior changes by consumers, and innovations in agritech and food formulations by businesses to make farm production more efficient and products more nutrient dense.

En el embarazo el requerimiento energético y de nutrientes aumentan, el déficit o el exceso puede afectar el estado nutricional de la madre y del neonato. Con el objetivo de determinar la frecuencia de malnutrición y su relación con complicaciones en mujeres embarazadas y sus recién nacidos que acuden al Hospital Distrital de Capiatá del Ministerio de Salud Pública y Bienestar Social, en el mes de febrero a agosto de 2022, se realizó un estudio observacional descriptivo de corte transverso retrospectivo que incluyó a 140 gestantes, de las cuales 64 presentaron complicaciones por malnutrición y 76 no presentaron ninguna complicación. La edad media fue de 26,9±6,6 años con un rango de edad de 15 a 40 años. Los resultados muestran que el 45.7% (n=64) de las gestantes presentaron complicaciones por malnutrición, el 51,6% presentó diabetes gestacional, anemia en el 29,7%, el 7,8% presentó anemia y diabetes gestacional, el otro 7,8% hipertensión arterial, el 1,6% manifestó hipertensión arterial y diabetes gestacional y el otro 1,6% presentó anemia, hipertensión arterial y diabetes gestacional. En relación a la evaluación nutricional al inicio y al final de la gestación en las gestantes con complicaciones hubo una leve disminución en los porcentajes de obesidad de 43,7% a 37,5% y un aumento del bajo peso de 11% a 20,3%. En cuanto al tipo de parto en las gestantes con complicaciones el 51,6% fue por cesárea (n=33), las cuales presentaron diabetes gestacional en el 54,5%, seguido de anemia en el 21,2%. Se comprobó un mayor porcentaje de sobrepeso y obesidad (29,7%) en los recién nacidos de madres con complicaciones. Se pudo evidenciar la frecuencia de malnutrición y complicaciones en casi la mitad de las gestantes, es preciso generar mayores estrategias en salud pública para prevenir la malnutrición y complicaciones en gestantes.


In pregnancy the energy and nutrient requirement increases, the deficit or excess can affect the nutritional status of the mother and the neonate. In order to determine the frequency of malnutrition and their relationship with complications in pregnant women and their newborns who go to the Capiata District Hospital of the Ministry of Public Health and Social Welfare, in the month of February to August 2022, a Descriptive observational study of retrospective cross -section that included 140 pregnant women, of which 64 presented complications for malnutrition and 76 did not present any complication. The Middle Ages was 26.9 ± 6.6 years with an age range of 15 to 40 years. The results show that 45.7% (n = 64) of pregnant women presented complications for malnutrition, 51.6% presented gestational diabetes, anemia in 29.7%, 7.8% presented anemia and gestational diabetes, the other 7.8% arterial hypertension, 1.6% expressed arterial hypertension and gestational diabetes and the other 1.6% presented anemia, arterial hypertension and gestational diabetes. In relation to nutritional evaluation at the beginning and at the end of pregnancy in pregnant women with complications there was a slight decrease in obesity percentages of 43.7% to 37.5% and an increase in low weight of 11% to 20, 3%. As for the type of childbirth in pregnant women with complications, 51.6%was due to caesarean section (n = 33), which presented gestational diabetes in 54.5%, followed by anemia in 21.2%. A higher percentage of overweight and obesity (29.7%) was proven in mother's newborns with complications. The frequency of malnutrition and complications could be evidenced in almost half of the pregnant women, it is necessary to generate greater public health strategies to prevent malnutrition and complications in pregnant women.

ABSTRACT Background/Aim: Chronic kidney failure is frequently related to malnutrition. This research aimed to assess the nutritional status of hemodialysis patients by assessing their biochemical and anthropometric parameters and determining whether the disorders suffered stemmed from nutritional deterioration directly related to time on dialysis. Materials and Methods: This research monitored 90 patients of both genders with chronic kidney failure who regularly received hemodialysis at the kidney unit of our Hospital in Granada (Spain) over five years. The patient's blood was tested quarterly for plasma albumin (Alb), total cholesterol (TC), and total proteins (TP) and monthly for transferrin (Tr). Anthropometric measurements were taken of the patient's weight, height, and body mass index (BMI) and, based on the patient's BMI, classified as established by the World Health Organization. Results: During the five years of our study, patients experienced a statistically significant decrease in total protein (0.941g/dl), plasma albumin (0.9382g/dl), total cholesterol (23.77mg/dl), and transferrin (78.17. g/dl) p < 0.0001. On the contrary, the mean BMI values did not show statistically relevant differences (p < 0.805). However, all patients remained in the WHO category of overweight. The body volume values did not show statistically significant differences either. Conclusions: In conclusion, the nutritional deterioration of these patients was not reflected in their BMI but mainly in their serum chemistry.


RESUMEN Antecedentes/Objetivo: La insuficiencia renal crónica está relacionada frecuentemente con la malnutrición, afectando aproximadamente a un tercio de los pacientes con enfermedad renal avanzada, lo que contribuye a su morbilidad y mortalidad. El objetivo de esta investigación fue evaluar el estado nutricional de los pacientes en hemodiálisis valorando sus parámetros bioquímicos y antropométricos y determinar si los trastornos que padecían se debían al deterioro nutricional directamente relacionado con el tiempo en diálisis. Materiales y Métodos: Es esta investigación realizó un seguimiento de 90 pacientes de ambos sexos con insuficiencia renal crónica, que recibían hemodiálisis periódicamente en la unidad renal de nuestro Hospital en Granada (España) durante un período de cinco años. La sangre de los pacientes se analizó trimestralmente para albúmina plasmática (Alb), colesterol total (TC) y proteínas totales (TP), y mensualmente para transferrina (Tr). Se tomaron medidas antropométricas de peso, talla e índice de masa corporal (IMC) de los pacientes y se les efectuaron mediciones antropométricas de peso, altura e índice de masa corporal calculado mediante la formula peso/talla², y agrupada según la clasificación de la OMS en IMC < 18.50 infrapeso, 18.50 a 24,99 normal, 25 a 29,99 sobrepeso y >30 del IMC s/OMS y se consideró para el estudio como desnutrición un en IMC < 23 kg/m2 y niveles de albumina <3,8 g/dl según el consenso del panel de expertos de la International Society for Renal Nutrition and Metabolism. Resultados: Durante los cinco años de nuestro estudio, los pacientes experimentaron una disminución estadísticamente significativa de proteínas totales (0,941 g/dl), albúmina plasmática (0,9382 g/dl), colesterol total (23,77 mg/dl) y transferrina (78,17. g /dl) p < 0,0001. Por el contrario, los valores medios del IMC no mostraron diferencias estadísticamente relevantes (p < 0,805). Sin embargo, todos los pacientes permanecieron en la categoría de sobrepeso de la OMS. Los valores de volumen corporal tampoco mostraron diferencias estadísticamente significativas. Conclusiones: La desnutrición de los pacientes en diálisis es un hecho patente, el IMC no se corresponde con los parámetros bioquímicos observados, por lo que el deterioro nutricional de estos pacientes se manifiesta principalmente mediante los parámetros bioquímicos estudiados.

ABSTRACT

This study investigated the relationship between dental hygiene and child maltreatment, malnutrition, and overall development. The researchers examined 900 students aged 5-15 years from government schools in Lucknow district, Uttar Pradesh. The study observed nutritional status by assessment of the physiological status of the body based on its height and weight and signs of abuse/neglect. The results showed that 260 abused students had poor dental hygiene, and 236 of them were mild to moderately malnourished. In comparison, 362 non-abused students had dental neglect, and 232 were moderately malnourished. These findings highlight a significant association between dental care neglect, child maltreatment, and malnutrition. It is crucial to address the social stigma surrounding this issue, normalize discussions, and encourage dentists to identify signs of abuse and inform relevant authorities about potential discrepancies.
